Passport: Original Passport with the validity of minimum six months from the date of return and minimum two blank pages for visa stamping.
Please Note: Attach first page, last page & observation page of passport along with all old passports (if any).
Philippines Visa Application Form : One visa application form to be duly filled and signed by the applicant.
Photo Specification: Two recent passport size photographs with matt or semi matt finish, 60%-80% face coverage, white background and without border (Size: 35mm x 45mm).
Please note: Photograph should not be more than 3 months old, scanned/stapled and should not be used in any of the previous visas.
Covering Letter : Covering-letter from company on its letter head stating the applicant’s name, designation, passport number and purpose & duration of visit. The letter should be duly signed by the authorized signatory and addressed to-“The Visa Officer,Embassy of the Republic of Philippines, Delhi”.
Invitation Letter: Invitation Letter from the shipping / inviting company stating the applicant’s name, passport number, designation, purpose & duration of visit. The letter should be duly signed by the authorize signatory.
Letter of Guarantee: A letter of guarantee from shipping company in Phillipines has to be submitted along with the following documents.
a) Valid seaman’s record book.
b) A copy of CDC (Continuous Discharge Certificate).
c) Appointment Letter / Contract.
d) For Supernumeraries: Proof of Relation to Principal Seaman.
Financials: Bank statement for last six months with 1,25,000 closing balance.
ITR: Last 3 years ITR OF applicant with BARCODE
Ticket: Confirmed air ticket.
Please Note:
a) If employed, a Certificate of the current employment from employer stating employer’s full name address and telephone no., applicant’s length of service and salary details is also required.
b) W.e.f 23rd April 2012, The Visa Fee for Same Day will be submitted in morning along with the application and even if Philippines Embassy is not able to issue Visa on the Same Day,the fee will not be refunded.
In case of a married couple travelling together Marriage Certificate is required to be attested by HOME+MEA+EMBASSY and embassy can also ask the same for any couple at random.
If Children below 15 years not accompanied by the Parents then VEG is required to be attested from HOME+MEA+EMBASSY.[Note: VEG is a NOC Letter signed by both the parents on a Stamp Paper attested by Local Govt].
If Spouse is Philipino then Spouse Name should be indicated in the Visa Applicants passport copy and report of Marriage issued by the Philippines Embassy or Philippines Govt.
